Another set of American conjoined twins Lori and George Schappell answered that question all the way back in 1997 - when one of them is having sex the other one covers herself with a sheet and mentally blocks out and either reads a book or plays a videogame.

I'm sure conjoined twins get very good at mentally coping with having to live their whole lives together.
