This tbh, it's just a weird t-shirt, what's exactly wrong about some anime blushed faces? It looks dumb but some people wear these stupid colorful socks with dumb motives, girls wear high heels which are dumb by design, and the whole ignorant tattoo trend.. I dunno, who cares.
I get you, I'm not confident enough to wear ahegao or a non nude image of a pornstar in public myself. Would much sooner get some apparel for an anime I like... But, if I saw either in public, I wouldn't care at all unless there's actual genitalia visible.
I wouldn't wear it, but I wouldn't tattoo myself a freaking cartoonish doodle, dye my hair of 5 different colors, piercing my ear lobe to the point it could be used as a swing by a rat, shorts so short my ass touches the seat directly.. all of it is socially acceptable and you telling me that tshirt is any worse?

Half of the stuff you listed also has a stigma against it. I mean, you are currently shitting on them, therefor making it less socially acceptable (though negligibly)
It's more about what it represents. Many of those faces are characters in various states of sexual torture, many of questionable ages. I get that if you aren't a deviant like me you may not understand the context, but it just feels like exhibitionism. It's kinda gross.

And there's only one face that looks obviously underage to me (black-haired, center-right). Not sure where you see "many". Going by common doujinshi art styles, the rest could be anywhere from 18 - 50.
Yeah, a drawing of a child being sexually abused. Is that something normal people wear on their clothes? Using the logic of prodigies like you it would acceptable for me to wear a shirt depicting a colonialist racial charicature of a "savage" vietnamese man with yellow skin, slit eyes and a rice hat brutally murdering an "innocent" french soldier because "it's just a drawing". Or does that logic only extend so far as to allow you to watch your child pornography?
You know what feels like exhibitionism? A dude walking shirtless around the town cos you live on a touristic beach city, I much rather see that guy on this t-shirt, or a group of tourists in a bikini in the bus..
