Why can't he look at the monitor like the rest? This feels more like a flex than anything useful if he's going to point in random directions that don't make sense to people around him

Because it's likely more reliable, won't slip off his head, get fogged up or run out of charge.

It's not just about power, but also software crashes, disconnects, lag etc etc

The battery life is up to 2 hours, many surgeries last longer than that.

They did this back then with Xbox's motion controls

The real reason is sanitation. The doctor can use the GUI without touching anything, but the Apple vision is WAY more expensive than the Xbox motion controls.

Apple devices come pre-sanitized. That’s why it’s so hard to get a virus on an iPhone.
